Criminal Background Check

 
The Wisconsin Caregiver Law requires MATC to check your criminal background if you are applying for admission to the MATC programs listed below. The fee for this service is $15. Your admissions application will not be processed until you have paid this fee. In most cases, you will pay the fee only once. As required by state law, a second $15 criminal background check fee may be charged if your educational program at MATC extends beyond four years.

All students applying for the programs listed above are required to complete and submit criminal background check forms. Forms are available at any MATC Admissions Office at each of the four MATC campuses or can be downloaded by clicking here. Please follow the instructions on the forms and read these documents carefully.
