Athletic Eligibility Form (PDF)

Welcome to MATC Athletics. We are glad to have you on our team.

All student athletes must complete the athletic eligibility form (PDF) and return it to the Athletic Department immediately.

Please keep the following in mind:

Physicals - You MUST have a physician's clearance before you can practice or compete in athletics. You may do this in one of two ways:

Go to your own physician. You don't need to complete a form, but your physician must indicate on their stationery that you are cleared for athletic participation. Fax this document to the Athletic Department at 414-297-6657, or give it to your coach.

- OR -

Bring your completed eligibility form to the Athletic Office to obtain authorization for a free MATC-sponsored athletic physical. You must have a completed form to get the free physical. Upon completion of your physical, please bring a copy to the Athletic Office.

Transcripts - If you attended another college before MATC, you must provide an official copy of your transcripts to determine your athletic eligibility. Fax your transcripts to the Athletic Department at 414-297-6657. If you turned in official transcripts at the Mequon, Oak Creek, or West Allis campuses, please note this on your eligibility form. If you submitted them to the Downtown Milwaukee Campus, please indicate that on your eligibility form.
